· 1 passport photo in colour, size 35 x 45 mm (head must measure 30-36 mm from chin to top of head), well-lit, on white or light grey background. Must be full frontal of the face and the applicant’s mouth must be closed. Must appear without sunglasses or toned/reflective spectacles. The photo must be up to date with the applicant’s appearance
· 1 application form (form J) properly filled in and signed by applicant. Must be latest edition of the form. This can be downloaded from the Embassy’s website www.ambsingapore.um.dk/en.
· 1 supplementary form (form J1) properly filled in and signed by applicant. Must be latest edition of the form. This can be downloaded from the Embassy’s website www.ambsingapore.um.dk/en.
· Valid passport. Original must be presented. Must be valid for at least 3 months after the intended stay in Schengen. Seamen must also present valid Seafarer’s Identity Document / Discharge Book.
· Valid visa/residence/work permit/study permit to Singapore. Original must be presented upon submission of application. If residing in Singapore the permit must be valid for re-entry to Singapore for at least 3 months after the intended stay in Schengen.
· Flight itinerary (confirmed booking of return flight). If travelling around in Schengen a detailed travel itinerary must be provided.
· Travel medical insurance (copy of policy and terms must be presented no later than upon collection of visa!). Applicant must have travel medical insurance covering any expense which might arise in connection with repatriation for medical reasons, urgent medical attention and/or emergency medical treatment. The insurance must be valid throughout the entire Schengen area. Minimum coverage shall be EUR 30,000 (approx. SGD 65,000 – may vary with currency fluctuations). The insurance must be valid for the entire period of stay. Corporate group insurances must comply with the above and employer must state that applicant is covered by corporate group insurance. Applicants are advised to bring original or a copy of the insurance policy (and a letter from employer if it’s a corporate group insurance) as it is a requirement for entry into the Schengen area. Lack of insurance may result in rejection of entry into the Schengen area.

Business:
· Letter from employer containing information on applicant’s current position and employment date. Must also state the travel period, number of days stay in Schengen required and how costs of the applicant’s stay in Schengen are covered (by employer or inviting company). For employees covered by group travel medical insurance the employer must state that the specific employee is covered and that the coverage is sufficient (according to the above note on travel medical insurance).
· Letter of invitation from company in Sweden. Invitation must be sent directly from the inviting company to the Embassy either by fax +65 6253 3764 or e-mail sinamb@um.dk.
· Appendices A and D must be properly and completely filled in by applicant.
· Hotel reservation or other lodging. This information must be provided – in writing - by inviting company, employer or the hotel in question.
Tourism:
· Hotel reservation or other confirmed booking of lodging.
· Documentation of financial means during the stay and covering of travel expenses, e.g. personal bank statement of applicant. As a general rule adequate per day amounts are SEK 370.00. If lodging has been paid in advance this will be taken into consideration. If staying with friends or family the applicant must prove to be in possession of own means to be able to socialize, use public transport, do shopping, visit tourist attractions and the like.
· Appendix D must be properly and completely filled in by applicant.
· Letter from employer stating period of employment and confirming that the applicant can return to his/her job after granted leave.
Visit to family or friends:
· Letter of invitation with details of the inviting person and the applicant. The relation to the applicant must be explained (family, fiancée, boy/girl friend, friend) and if fiancée the length of the relationship must be stated.
· Copy of civic registration certificate (personbevis) of inviting person. This can be downloaded from www.skatteverket.se or ordered from the nearest tax office.
· The inviting person must complete Appendix E and return it to the applicant so that it may be submitted along with the application.
· Appendix D must be properly and completely filled in by the applicant.
